If you believe you are entitled to missing royalties, please contact the US Royalties Hotline at 615-726-8326.

If we determine that you are missing royalties, you will be required to complete a Change of address form and W9 form (US Residents only; non-U.S. residents please complete a W8-BEN form).

Once the forms are completed, please return them to Courtney Eason at 8 Music Square West, Nashville, TN 37203, courtney.eason@sonyatv.com. or fax to (615) 743-1966. Make sure to notify us that you believe you are missing royalties, and that this is not just a standard address change.
