Change of Address Policy & Procedure

Company policy requires a client to complete a W9 and Change of Address Form to formally change their address in our system. Please send back both completed forms to Meghann Langford at Sony/ATV Nashville. These forms must be completed legibly or we cannot update your information.

Please note that the W9 form (US residents only) is for tax purposes and will reflect the information on your check if you are eligible for payment. Your statement and check (if applicable) will be mailed according to the information on the Change of Address form. If you are a non-U.S. resident, please complete the form W8-BEN.

Setting up an Estate or Beneficiary Account

The following documents are required to set up an estate or beneficiary account:

Copy of death certificate
Copy of Last Will & Testament, Letters Testamentary, or Court Probate Document. This should show the sole beneficiary, or all executors of Estate that will receive monies or be in charge of account.
Completed W9 for payment and tax purposes.
